What is Failure Mode and Effects Analysis

In general Failure mode and effects analysis (FMEA) is a systematic method for identifying and addressing potential failures in Assets before they occur. It's a tool that can be used to evaluate new or existing Assets, and to help prevent undesirable failures. 

Here are some key aspects of FMEA:

•    Identify failure modes: 

Review components, assemblies, and subsystems to identify potential failure modes

•    Consider the effects: 

Determine the potential consequences of each failure. 

•    Rate the risk: 

Assign a score to each failure mode based on its likelihood of occurrence and impact. 

•    Prioritize: 

Use the risk rating to prioritize which failures to address first. 

•    Prevent failures: 

Take action to improve and prevent failures. 

FMEA is similar to the everyday practice of anticipating what might go wrong and taking steps to prevent it
